Resident Service Coordinator Program

All of our communities offer Resident Services programs that support older adults and residents with disabilities to help them remain stable and independent in their homes. The program connects residents to services and benefits, builds supportive on-site programming, and serves as a liaison among residents, property management, families, and community providers.

Both Evergreen House Apartments in Annandale, Virginia, and Marywood Apartments in Manassas, Virginia, offer full-time dedicated Resident Services Coordinators and a more robust set of services.

Key Components Typically Include:

Individual Support, Referrals, and Advocacy

Helping residents access community resources and entitlement benefits (federal, state, and local) and advocating with public/private organizations as needed. The Service Coordinators specialize in networking for services within the property’s geographical communities.

Education and Empowerment

Offering educational programs on topics like community services and Social Security benefits so residents can better navigate support on their own.

Community-Building & Wellness Programming

Addressing residents’ social and recreational needs through activities that promote “aging in place.” The properties offer exercise programs, health clinics for preventative vaccinations, and screenings, along with speakers and a variety of health-related activities.

Volunteer Coordination

Developing volunteer opportunities to support frail and at-risk residents and creating ways for the broader community to engage with residents.

Collaboration with Property Management

Educating the management team on aging-in-place issues and available resources to improve resident support and outcomes. Management holds quarterly company-wide training sessions for Service coordinators and site management, with topics and speakers focused on seniors’ needs and creative approaches to support the aging process.

Communication and Outreach

Keeping residents informed about existing and new services via newsletters, mailings, and bulletin boards.

Home Visits and Transportation (When Applicable)

Conducting home visits for homebound residents and transporting residents to approved social, health, and educational activities.

Documentation and Reporting for Marywood and Evergreen

Completing resident assessments, maintaining secure resident files, and preparing required reports (e.g., HUD semi-annual Service Coordinator reports and monthly resident services reports).

24-Hour Front Desk at Evergreen

Evergreen offers 24-hour concierge service for our residents that ensures limited, secured access with intercom and video surveillance, allowing the community to safely age in place. This environment has fostered a thriving “blue zone” where people feel safe and lead active lifestyles.

Transportation

Each community is equipped with a shuttle service and a driver, offering transportation to the grocery store, shopping, for field trips and to the senior daycare center. Partnerships with various nearby faith-based organizations offer pick up and drop-off to various houses of worship.

Amenities

All of our communities include numerous amenities that improve daily life for seniors, such as an on-site beauty salon, a fitness center, a community room, a computer area, game rooms, library and central laundry. Outdoor amenities include community gardens, attractive gazebos and gathering spaces.
